PHARMACY INQUIRIES

Physician’s fax and billing information is available here, and contacting us by fax is preferred.

If you have questions regarding a faxed prescription, or need us to re-send a prescription or clarification fax, please request this via fax. When doing so, please date your request, and include the patient’s name, OHIP number, and date of birth.

If you would like to request a prescription renewal on behalf of a patient, please fax the request to our office.

THIRD PARTY INQUIRIES

If you have questions, need clarification, or need a referral / requisition re-sent to your office, please send us a fax with your question/request. When doing so, please date your request, and include the patient’s name, OHIP number, and date of birth.

If you are requesting patient medical records, please also include a signed consent form.

PHYSICIAN TO PHYSICIAN MEETING

If you would like to set up a meeting/phone call with one of our physicians regarding a mutual patient, please call our office and provide the following information:

  • Who you are

  • The physician you would like to correspond with

  • The patient it is regarding (last name, first initial and date of birth)

  • What it is regarding

  • Some dates and times that you would be available
